Are you passionate about supporting your community? About the role We are seeking a dedicated

Case Manager for our Family Housing Support Program! You will be responsible for the provision of high-quality case management support within the Family Housing and Support team to families and children at risk of homelessness. Responsibilities include Support implementation of the Homelessness Stream Model of Care that underpins services across the country and defines expectations Ensure suitable referrals are sent to internal and external agencies in a timely manner that promotes streamlined connections Ensure case notes and work completed adheres to accreditation standards Establish and maintain strong relationships with key internal and external stakeholders to continuously improve referral pathways Deliver strength based, trauma informed and client focused approach

This is a permanent full-time position, based in Sunshine, VIC. Salary and conditions are in accordance with SCHADS Award Level 5. You will have Tertiary qualifications in social work or similar field Experience of working with external stakeholders in providing a multi-task service delivery (desirable) Previous experience in providing relevant client centred and strength based assertive case management approach Relevant experience in planning, implementing and evaluating actions required to meet families housing needs Experience in identifying relevant sources of independent advocacy pathways for families Knowledge of community development and networking principles and approaches Previous experience working within homelessness sector working with families A current and valid Victorian Driver's Licence Benefits of working at
