Form of workWe are seeking an experienced Case Manager (Enrolled/Registered Nurse) to support our Aged, Disability and clients with severe personal injuries located in Sydney. Ideally, you will have a strong mix of case management and nursing skills and enjoy working with clients in the community.
Your role will utilise your nursing and case management experience and will include the provision of nursing procedures, training and supporting field staff, managing client intake, assessments and care planning, case management, managing client budgets, as well as coordinating and monitoring all aspects of service to ensure clients' needs are met.

What is required?
- Case Management experience in Aged Care / Disability / severe personal injuries
- Sound understanding of NDIS/Home Care Packages
- AHPRA Registered/Enrolled Nurse
- Superior interpersonal and written/verbal communication skills
- Exceptional customer service skills
- Ability to build strong relationships and networks within the community
- Valid Australian driver's license and access to a reliable car with comprehensive insurance
- A current National Police Check

The salary package is negotiable and will include a car allowance and the opportunity to earn performance-based bonuses.

Our team of support workers are required to complete a comprehensive recruitment process and are selected based on their understanding, compassion, and experience in providing quality care services. We then match each carer with clients based on personality, skill set, and experience. We continue to educate and support our carers with ongoing education and training, all overseen by our experienced nurses.
