MSS has an opportunity for a permanent, full-timeHome Care Package Case Manager within our Home Care Program based in Taree. The position is 38 hours per week Monday-Friday (with the option of a 9-day fortnight).
Our Case Managers work closely with MSS clients, their carers and families to plan and coordinate support services in line with client needs and goals, ensuring our clients can stay living independently in their own homes with the dignity and respect they deserve.

Selection Criteria
Qualifications and Requirements
- Completed a Tertiary Management Diploma, Certificate III or IV in Individualised Support, Home & Community Services, Aged Care, Disability or related relevant experience.
- Current drivers licence (not Ls)
- Minimum third-party property damage insured vehicle
Skills and Experience
- 12 months Case Management or Senior Coordinator/Team Leader experience
- Excellent problem-solving ability for complex issues and the ability to make appropriate decisions.
- Excellent assessment and care planning skills
- Working knowledge of Home Care Packages and the Aged Care Standards
- Experience in assisting people to make choices enabling them to reach their full health and well-being potential.
- Outstanding interpersonal & communication skills, both written and verbal
- High-level administrative skills including the proficient use of Microsoft Office programs and willingness and ability to use client databases.
- Understanding the needs of people from a diverse range of backgrounds
