Robina State High School is seeking an enthusiastic, experienced Inclusion specialist to join our team as Case Manager of our inclusion students for the start of Term 3 for the term. The role does have the possibility of extension beyond Term 3.
The Case Manager is the lead teacher around curriculum, communication, social participation/emotional well-being, health, personal care and learning environment access for diverse learners captured in the NCCD in a particular phase of learning. The Case Manager embraces culture through creating inclusive teaching and learning environments. They will identify and provide opportunities to overcome barriers, realising the potential of every student.
LEAD ROLE WITH STAFF INCLUDING:
- Responsibility and accountability for the nominated students in their assigned phase of learning.
- Oversee the development of the ICP and ensuring that relevant instruction and services are provided to the student to optimise their learning and inclusion.
- Provide input and specialist advice to all teachers to support the differentiation of planning, teaching and assessing of students.
- Provide input and specialist advice to teachers who have case managed students in non-co-teaching settings.
- Utilise experience and knowledge of disabilities to develop and support staff across the school to provide inclusive learning environments.
- Manage, plan and implement student learning pathways relevant to the phase of learning. This includes QCIA senior pathway, work experience, career pathway and subject selection processes.
- Regular communication with Support Staff to support case managed students.
COMMUNICATION / LIAISON ROLE
- Key point of contact for parents and external agencies.
- Build strong relationships with the students and their families.
- Reports regularly to parents through PT interviews and additional scheduled meetings.
- Consult/liaise and refer to EQ therapy services, outside agencies or professionals and support personnel to facilitate the student’s access to an education.
- Meet regularly with the Phase of Learning teams to ensure appropriate tracking, monitoring and intervention processes are taking place to support student improvement.
SUPPORT ROLE FOR STUDENTS
- Go to person for the student.
- Facilitate transitions in relevant phase of; Primary → High School, Junior → Senior Schooling, High school → post school.
- Regular contact with the students in the playground and in their classes and have expert knowledge of their learning and social emotional needs.
- Facilitate and support social/emotional well-being and extra-curricular participation.
- Facilitate alternative assessment and examination conditions including Access and Reasonable Adjustments (AARAs) in conjunction with HOD Inclusion and Curriculum HODs.
- Provide one-on-one or small group assistance / instruction for assessment and learning if required.
- Liaise with ILO to facilitate work experience / TAFE/ School Based Traineeships. (Y10-12)
POLICY IMPLEMENTATION
- Write, communicate and maintain all aspects of personalised Learning Plans (PLP) for identified students.
- Advocate for application for Special Consideration through Queensland Curriculum and Assessment Authority (QCAA) and communicate with relevant classroom teachers.
- Complete and maintain records with HOD Inclusion associated with NCCD for nominated students.
- Collaborate/consult with other school support personnel.
FOCUSED INTERVENTION
- Tracking of student progress through analysis of data including attendance, achievement, behaviour and effort.
- Facilitating stakeholder meetings with key support personal including external specialists to inform support planning and supportive strategies towards positive student outcomes.
- Collaborating with and developing the capability of Phase of Learning Teams and classroom teachers to implement supportive strategies towards positive student outcomes.
Time Allocation:
Teach 2 x lines (420 mins) across the week.
