Your opportunity:
- This is a case management and clinical service delivery position incorporating assessment, care planning, coordination, and treatment of clients during their episode of care whilst on the Transition Care Program.
- The Health Practitioner provides direction and support to other staff while promoting professional standards of practice and conduct and, is responsible to contribute towards the achievement of unit goals and standards. The Health Practitioner may be allocated portfolio responsibilities to facilitate this.
Temporary Part Time (38 hours p.f.) from 16th September 2024 until 17th March 2025, with the possibility of extension.
Prior to being made an offer of employment, you will be required to demonstrate immunity to the following Vaccine Preventable Diseases (VPD): Hepatitis B, Measles, Mumps, Rubella, Varicella (chicken pox) and Pertussis (whooping cough).

You will receive a competative salary between $2,941.70 - $4,323.80 p.f., employer contribution to superannuation (up to 12.75%) and annual leave loading (17.5%) (HP3) (Temporary Part Time, 38 hours p.f., from 16th September 2024 until 17th March 2025, with the possibility of extension).
