The Opportunity
Aveo Care at Home is an approved provider of Commonwealth-funded Home Care Packages (HCP) to residents throughout our retirement living communities across Australia. Our goal is to deliver tailored care and support that helps residents remain independent in their own homes for as long as possible. As a Case Manager, you will be responsible for assessing the care needs of our residents, providing support and guidance to access government-funded services, and managing Home Care packages. You will work closely with residents to develop personalised care plans, coordinate services, and ensure compliance with quality standards. This is a rewarding opportunity that will enable you to actively achieve quality of life for residents while working within a supportive and experienced team.
This is a permanent full-time position working Monday-Friday and will work with residents across our Clayfield, Newstead and Robertson retirement villages. Vehicle provided for your daily travels!

You will:
- Meet with prospective Aveo residents and their families to understand and assess their care needs
- Provide tailored information on care needs, services and funding options available
- Establish Home Care plans aligned to resident care needs, ensuring supports are in place as required
- Prepare and maintain records, documents and reports as required
- Deliver excellent customer service to residents, families, staff, management, visitors and external health professionals
- Monitor the effectiveness of services by ensuring consumers receive the care and support services they require to meet identified needs, goals and preference
- Liaise effectively with external service providers who provide care within the community
- Promote Aveo Care at Home services by attending community events, assisting with the transfer of Home Care packages and provide support throughout the My Aged Care journey
What you will bring:
- Current AHPRA Div 1 registration with experience preferably in a Home or Community Care setting
- Demonstrated experience in understanding and managing care plans and clinical assessments
- Solid understanding of Home Care Packages or similar commonwealth funding package models (e.g CHSP or similar)
- Prior experience in case management and/or exposure to My Aged Care or ACAT Teams (desired)
- Excellent people management, technology and communication skills
- Self-motivated individual with a practical and professional approach to case management
- Current Drivers License and two (2) COVID Vaccinations (required)
