AddressBrightside Recruitment is representing a medium sized Not-for-Profit Specialist Youth & Homelessness Service, providing services to children and young people aged 9 and 24 years old.
Role:
Based on the North Shore, this is a role not to be missed. We're looking for experienced and enthusiastic Case Managers with experience of working with children and young people, and people at risk of homelessness.
As a Case Manager you will bring your experience and skills to the program, ensuring the provision of quality support and accommodation services to young people and their families. You'll be involved in referrals, assessments, general support, community placement, supported accommodation, outreach, and aftercare services with a caseload of 4-8 young people.
The Role:
- Manage an intensive case load of 4-8 participants, depending on case complexities
- Complete intake assessments with new participants, making relevant referrals to other providers
- Provide person centered, strengths based general psychosocial support
- Help build life skills and capacity to participate in education programs, employment and community events.
- Liaise with and advocate for participants with relevant stakeholders
- Minimum requirement of a Diploma/Bachelors Degree in a relevant field
- Ability to work towards case management and therapeutic models
- Ability to manage crisis and critical incidents and work with challenging behaviours
- Knowledge of the issues faced by children and young people in the Out of Home Care and wider Child Protection sectors.
- Understanding of legislative and compliance requirements of providing services to children and young people at risk of homelessness
