Directions Health Services is an innovative non-profit organisation that has provided specialist alcohol and other drug (AOD) services in the ACT and NSW for more than 40 years. The Sapphire Health & Wellbeing Service (SHaWS) is one of Directions’ programs. SHaWS provides non-judgemental and respectful AOD treatment, alongside integrated primary health, mental health and Aboriginal health services to clients in the beautiful Sapphire Coast region (Yuin-Monaro Country).
Our Directions’ team includes Counsellors, Social Workers, a Psychologist, Chemists, GPs, Registered Nurses, and a Psychiatrist. Our capacity to implement innovative services developed in collaboration with our clients, partners, and other stakeholders, enables us to effectively respond to the changing needs of the communities we serve.

This is a fixed term, full-time position with a contract until June 2027. For the right applicant, a part-time role will be considered.
About the position
As a Case Manager within the multi-disciplinary SHaWS program, you will provide culturally responsive and coordinated community-based treatment including case management and support to clients and family members impacted by alcohol and other drug issues. You will also provide post-custodial support to clients recently released from custody or planning for release from custody. To provide these services you will work closely with Directions’ programs and consortium partners, as well as local health, community, housing, and justice related agencies.
Other responsibilities
  • Work with clients, utilising a stepped model of care to identify goals and develop holistic individual treatment and support plans
  • Make pre-release contact with people due to exit custody to build a relationship with them and assess their needs prior to release
  • Create positive relationships with the relevant correctional facility and Probation and Parole staff to facilitate pre-release contact
  • Undertake routine outcomes measurement in collaboration with clients
  • Ensure accurate and timely collection of required data and ensure documentation including client case notes, reports and other written communication is well maintained for effective handovers when necessary
  • Promote and contribute to a continuous quality improvement and safety framework
  • Maintain professional standards in relation to service delivery and confidentiality and work in accordance with health and safety guidelines
  • Participate in staff meetings, supervision, and shared care meetings
  • After hours work may be required

What we would like you to have
  • A qualification in Counselling/Social Work/Psychology/Nursing/another relevant field
  • A Cert IV in AOD or willingness to obtain, and experience working in AOD and Mental Health sectors
  • Experience working with or within the correctional /probation and parole sector is desirable
  • Experience and/or training in providing case management and counselling services
  • An understanding of the impact of AOD use and the associated issues this client group may experience, including physical and mental health and other co-morbidities as well as knowledge of effective AOD treatment and support options and harm minimisation strategies
  • Skills in using electronic client information management systems and Microsoft Office
  • Willingness to travel regularly within the Sapphire Coast region
  • Great communication, interpersonal, teamwork and organisational skills
  • A capacity to provide culturally appropriate, person-centred, and non-judgmental care
