Aboriginal or Torres Strait Islander applicants must demonstrate Aboriginal or Torres Strait Islander status in addition to addressing the selection criterion.The role:As a Clinical Nurse SpecialistCase Manager, you will play an important part in initiating, developing, implementing, and evaluating strategic changes, whilst having a lead role in quality activities, education development, research projects, and governance processes within the department. Your extensive experience in rehabilitation, coupled with exceptional communication skills, self-motivation, and organisational abilities, will contribute significantly to our patient care standards.The role of Case Manager necessitates overseeing the entire patient journey, from initial handover to discharge planning. You will prioritise tasks to meet KPIs and facilitate smooth patient flow within the hospital, emphasising early discharges where appropriate. Your leadership on the ward will set a high standard of nursing care and professionalism, essential in managing complex family circumstances and advocating for patients' needs. You'll contribute to the development of specialty clinical practice and service delivery in rehabilitation nursing, mentoring, and guiding less experienced healthcare practitioners and team members.Supported by the Nurse Unit Manager and Head of Department, you will provide individualised, case-managed complex patient care and leadership within rehabilitation to maximise health outcomes. Using your advanced clinical skills and professional judgment, you will assess, plan, organise, evaluate, and deliver specialist high-quality nursing care tailored to achieve optimal clinical outcomes for each patient.Where you'll be working:This is an opportunity to work for the largest employer in the region which services a population of more than 900,000 across the Hunter, New England and Lower Mid North Coast areas. John Hunter Hospital is a Level 6, tertiary referral, major trauma and teaching hospital and is the largest hospital in the Hunter New England Local Health District.The NSW Government has committed $835 million to redevelop the John Hunter and John Hunter Children's Hospital. The John Hunter Health and Innovation Precinct project will deliver an innovative and integrated precinct with industry-leading facilities. The project will work in collaboration with health, education, and research partners to meet the current and future needs of the greater Newcastle, Hunter New England, and Northern NSW regions.Benefits:

Requirements:
  • Current Authority to Practice as a Registered Nurse with AHPRA with relevant post-registration qualifications and at least 3 years experience working in rehabilitation, or such other qualifications or equivalent experience deemed appropriate by the public hospital or public health organisation.
