St Bart’s is a Western Australian not-for-profit organisation founded in 1963 as a small homeless shelter for men. For over 60 years, we have been standing by the side of vulnerable Western Australians experiencing, or at risk of, homelessness, mental health challenges, trauma, and hardship. Since our foundation, we have evolved into a larger organisation that provides progressive, person-centred, and trauma-informed recovery support to people in need, regardless of background, age, gender, or ethnicity.

About the role
Our Care Finder program assists people to access appropriate accommodation. Additionally, we support them with understanding and accessing My Aged Care services and connecting them with other relevant supports in their community. The program is specifically tailored to help older people living in insecure housing and/or experiencing homelessness.
The Case Manager is responsible for,
- Preparing care plans in consultation with the consumer using reablement and restorative care approaches.
- Conducting reviews of existing care plan to ensure that they still meet consumer needs.
- Making appropriate referrals to services and monitoring progression.
Taking into consideration the qualifications, skills, and experience of the successful applicant the base salary offered for this position spans from $82,043.52 to $88,287.68 plus superannuation.
About you
We are seeking a compassionate and personable individual with strong experience in the community sector and advanced case management skills. You need to be motivated to walk alongside people who have experienced Trauma, making a meaningful impact in their life.
In addition, our ideal candidate will have:
- A well-developed understanding of the operation of the aged care system, or ability to rapidly acquire this understanding.
- Ability to participate collaboratively and constructively with colleagues, senior leaders, and stakeholders, as well as being able to demonstrate effective decision-making skills.
- A tertiary qualification (minimum Diploma) in Community Services, Aged Care, or a similar discipline
- Ability to manage designated case load and ensure referrals are followed up in the appropriate time frame.
