We aim to grow multicultural values across our State and to empower newly arrived Queenslanders from diverse cultural and language backgrounds through advocacy, community development and a range of client services.

About the role:

Multicultural Australia is currently offering an opportunity for a Care FinderCase Worker to work as part of our Client Services department to deliver Care Finder Program. The role is Full time Maximum term (May 2025), based in Brisbane. 

Care Finder Program is to deliver services to older adults from Culturally and Linguistically Diverse Backgrounds (CALD) within the Care Finder target population. The role will provide Care Finder services to vulnerable older people in Brisbane South LGA area, requiring support to navigate the My Aged Care system and provide an assessment and referral point for connection with appropriate mainstream services and community. This will involve utilising an assertive outreach approach to engage and build rapport with potential clients as well as local intermediaries, stakeholders, and networks. 

Primary Responsibility:

Among other duties, the successful candidate will:

  • Deliver case management services that are culturally competent to seniors from a variety of refugee, humanitarian and migrant backgrounds in accordance with the Care Finder Program agreement, model, and timeframes;
  • Work closely with Care Finder Coordinator and Team Leader to support service delivery priorities and the coordination of our Care Finder services to be consistent in meeting requirements and quality standards;
  • Conduct a psychosocial needs assessment with each client and their family and create case management plans addressing individual aged care needs and goals;
  • Provide assistance to help individuals understand and access aged care services and connect with other relevant supports in the community, in a culturally sensitive and respectful way;
  • Provide appropriate referrals to clients to different/appropriate services that may assist the client to improve their wellbeing;
  • Maintain productive, cooperative working connections with important stakeholders to guarantee efficient service planning and delivery to clients, improve referrals and support team promotions of the Care Finder program by attending networks, community awareness activities, community events when directed;
  • In line with MA policy and procedures, collaborate with volunteers, CSWs, bicultural workers and interpreters to provide culturally sensitive and informed supports to Care Finder clients and their families;
  • Identify and escalate reportable situations and concerns to a line manager in accordance with incident reporting policy and procedure, and where a client, agency, staff member or organisation is at risk;
  • Conduct assigned administrative task including completion case notes in the case management system and timely reporting according to the requirements of the Care Finder reports;
  • Participate in the emergency on-call roster which involves responding to calls to the Multicultural Australia’s 24-hour emergency mobile phone periodically with support as needed of on-call manager;
  • Attend, and participate in, team meetings, team building and planning, and professional development activities; 
  • Other duties as directed by management.   

What qualifications and skills do I need?

You will be comfortable working in a fast-paced environment and have effective communication and interpersonal skills. As well as having a confident and positive personality, you will possess:

  1. Possession of Bachelor level tertiary qualification in the field of Social Work; Psychology, Applied Social Sciences, Counselling or other relevant allied health disciplines or Diploma of Community Services (Case Management). Other relevant qualifications (including overseas qualifications) or experience may be recognised;
  2. Relevant experience in a similar role, in case management, aged care, or in the not-for-profit sector will be highly regarded;
  3. Demonstrated stakeholder engagement skills in relation to both intra-organisational relationships (i.e., between program areas) and inter-organisational relationships (i.e., between collaborating organisations and agencies), including the ability to negotiate for outcomes that are beneficial to clients;
  4. Demonstrated experience providing administrative support in a fast-paced diverse environment;
  5. Excellent communication skills, in working with clients that have English as a second language and/or limited English skills, and in building strong team, stakeholder, and client relationships;
  6. Demonstrated experience in working with client management data systems;
  7. Experience in prioritising, problem solving and responding to competing demands in a calm, organized and timely manner within a fast paced and diverse work environment.; 
  8. Good computer literacy, particularly in relation to use of Microsoft Office Suite (Word, Excel, PowerPoint, etc.) and databases.

Successful candidate will be required to have a valid driver’s licence, police check and apply for a Working with Children Blue Card.
