Why this role matters The Salvation Army Australia’s Homelessness Stream provides high quality, person centred services to people at risk, and or experiencing homelessness. We support adults and families, including accompanying children with accommodation, case management, outreach support, financial assistance, connection and referral to other specialist services.
The Salvation Army Homelessness Services is seeking a First Response Case Worker to join our close-knit team!
You will bring your innovation, adaptability, resilience, networking and time management skills forward to provide person-centred support to those in our community who are reaching out to the First Response Team. Our
First Response Team works with individuals and families (embracing diversity of family make-ups) who are at immanent risk of or are experiencing homelessness. Our team consists of Case Workers across Brisbane City, Sunshine Coast, Gold Coast and now Moreton Bay. We work collaboratively to provide a comprehensive and resourceful response to participant needs.
This position will be predominantly based in Moreton Bay but will work in multiple locations across the region. You will successfully provide Initial Assessment and Intervention Answer calls in a timely manner, with clarity and in plain English, providing initial response and assessment, inclusive of needs, risks and vulnerabilities Complete comprehensive initial assessment and intake which promote participation, self-determination and connection to community Utilising person-centred and trauma-informed practice to understand immediate needs, complete referrals and respond appropriately to risk Employ well developed interpersonal communication skills to respond to community members presenting in person and over the phone Ensure timely and accurate records and notes Case Management Development of case plans alongside participants, which reflect their needs, goals and interventions to address barriers Continual assessment of presenting issues, vulnerabilities and needs from initial intake to exit Advocacy of participants' needs and vulnerability to stakeholders
Working in a mobile capacity to meet with community members and stakeholders where required You will have Tertiary qualification in Community Services and Social Work is desirable.
A degree level qualification is preferred. Relevant experience in a social service environment including in client assessment and case management. Prior work experience or training in Domestic & Family Violence highly regarded Prior work experience or training in Mental Health highly regarded Previous experience providing case management type support over the phone preferred.

This is full-time, permanent position located in Spring Hill, QLD. Salary and conditions are in accordance with the Social, Community, Home Care and Disability Services Level 2 Award. What we offer
