The Position
The Shire of Manjimup is seeking a suitably qualified and experienced individual to join the Shire’s Home and Community Care team. This role is responsible for:
- Assessing the needs of individuals and their service requirements whilst promoting physical and emotional wellbeing.
- Developing support plans for individuals residing in the Respite House and report required changes to the Clinical Care Coordinator or Manager Home & Community Care.
- Assisting with clinical aspects of HCP consumers when required.
- Ensuring Support Workers and EN’s under your supervision are appropriately skilled and understand the needs of the consumer.
- Administering schedule 8 medications and provide specialised nursing care as per consumers clinical support plan in the Respite House and in home care.
- Providing high quality supervision of the Respite House, ensuring the respite service runs effectively and efficiently on a day-to-day basis.
- Available for flexible hours during the day and weekends, including overnight on call shifts.
- Liaising with medical professionals, allied health professionals, co-workers and other personal in order to provide the best outcomes for individuals.
- Assisting with clinical training to Respite House and Community workers.
The Person
To be successful in this role you will need:
- Current AHPRA Registration as a Registered Nurse.
- At least 3 years nursing experience within the Aged & Disability care sector.
- Demonstrated advanced clinical experience and expertise working with consumers in the community.
- Understanding of care assessment in the Aged Care sector.
- Knowledge and experience with supporting people living with dementia or a disability.
- Excellent computer literacy and communication skills both verbally and written.
- Current 'C' class driver's licence and a registered reliable vehicle with full comprehensive insurance is required.
- Knowledge of Aged & Disability legislation and standards (desirable).
- Knowledge of reporting requirements as prescribed in the contracts with the relevant funding bodies (desirable).
