The Institute for Urban Indigenous Health Ltd was established by its founding members to provide a coordinated/integrated approach to the planning, development and delivery of comprehensive primary health care services for Aboriginal and Torres Strait Islander populations within the Southeast Qld Region.
Moblink’s aim is to improve access to the most appropriate health and social services for Aboriginal and Torres Strait Islander people at the right time and in the right place with a view to reducing avoidable acute hospital admissions or re-admissions and emergency department presentations; improved use of the healthcare system and to support people to take part in a healthy lifestyle and support clients to access social supports including safe accommodation
About the Role?
The purpose of the position is comprehensive linkage and coordination service to address gaps in care for Aboriginal and Torres Strait Islander people at the interfaces between home/community, primary care, and secondary and tertiary care services. Provide support to immediate team members to facilitate effective and safe case management. Support clients to address social determinants of health and Coordinate clients with complex needs between IUIH community and hospital services for all children, youth and their families using a family centred approach. This position aims to reduce significant points of risk in a person’s journey through the health care system, in particular, at hospital entry and discharge, and in transit from primary care in and out of specialist, allied health and other services.
Day to Day Duties may Include:
- Support team members in care provision to identified client base and provide case direction.
- Participate in regular Case Review meetings to support team collaboration in providing holistic care.
- Support with triage and allocation of referrals to Care Coordinators within parameter of scope of practice.
- Engage, collaborate and coordinate with other services which support children, young people and their families.
- Coordinate holistic supports for children, young people and their families.
- Provide support and information sessions to internal and external service providers to support and streamline client journeys.
- Collaborate with your own and other stakeholders through joint planning, sharing of information, advice and expertise.
- Travel across SEQ is required at times to meet client and service needs
About You?
- Minimum bachelor’s degree in nursing
- AHPRA Registration
- Current C Class Drivers Licence (Qld)
Desirable, but not mandatory:
- Demonstrated experience working with Aboriginal and Torres Strait Islander people and /or communities
- Care Coordination and Child and Family Health experience advantageous.
