The Community Hospital Interface Program (CHIP) is a discharge planning service that provides a link between the acute hospital setting and community service providers. Based within Ipswich Hospital, the CHIP service provides complex discharge planning for at risk older patients to facilitate safe, timely and sustainable discharge.

About the role

The CHIP nurse is an expert in the holistic assessment of older patients; is able to work collaboratively with the multidisciplinary team and is able to develop positive relationships with internal and non-government service providers. The CHIP service also incorporates the Sub and Non-Acute Patient (SNAP) Coordinator role on a rotational basis.

The Clinical Nurse is a registered nurse who is accountable for practice at an advanced level, working collaboratively within the unit and with other members of the health care team including consumers/carers, other health professionals, general practitioners and other agencies (including non-Government agencies). This role provides the delivery of clinical care and support specific to older persons within a hospital setting.

About you

Our ideal candidate will be someone who can demonstrate the following:

  • Demonstrated advanced clinical knowledge in the area of discharge planning  delivering expert nursing care in complex situations while providing support and direction to other team members.
  • Demonstrated ability to role model excellent standards of comprehensive, evidence-based nursing care and documentation to facilitate quality patient outcomes, in accordance with National Safety and Quality Health Service and professional standards.
  • Demonstrated ability to communicate effectively with members of the healthcare team, patients, relatives and visitors and maintain a positive workplace culture through well developed consultation, liaison, negotiation and conflict resolution skills.
  • Demonstrated ability to build and maintain strong relationships and a positive environment within a multidisciplinary team, actively contributing to the learning and development of others to a high standard.
  • Ability to prioritise clinical and related administrative duties, including effective and efficient planning and organisation of complex caseloads with proven ability to use information and reporting systems appropriate to the position.
  • Demonstrates honesty, integrity and respect for all patients, carers and staff.

Mandatory Requirements:

  • Vaccinated against, and remain vaccinated against, certain vaccine preventable diseases (VPD) as outlined in the Health Employment Directive NO. 01/16 (PDF) ( https://www.health.qld.gov.au/__data/assets/pdf_file/0009/150120/hed-0116.pdf )
  • Appointment to this position requires proof of qualification and registration or membership with the Australian Health Practitioner Regulation Agency. Certified copies of the required information must be provided to the appropriate supervisor/manager, prior to the commencement of clinical duties.
