Chronic Disease Management Rn/En

We are looking for an experienced EPC RN/EN to join The Family Practice at Glenmore. Ideally you will have solid Knowledge to undertake care plans as you'll be responsible for our EPC's (Chronic Disease Management plans)

For this position, we would require a Registered Nurse/Enrolled Nurse with:

• Experience with care plans a must
• General practice environment
• Chronic Disease Management plans
• Immunizations, wound care/dressings, and pre-employment checks
• Experience with Medical Director/Pracsoft (desirable)
