GHC is looking for a Client Intake officer to facilitate Client Intake, assessment and client referral, as well as providing the client with information about their mediation pathway, other support services, and being the interface between new clients and the support they receive.
Successful applicants will have the following attributes:
- A Diploma / Cert IV level qualification in a relevant field and/or an equivalent combination of relevant experience, education and training.
- Experience in working to uphold the best interests of those living with disability and the mandatory reporting.
- Well-developed organisation and priority management skills
- Experience in developing and maintaining productive working relationships with key internal stakeholders including a multi-disciplinary team.
- Well-developed interpersonal and communication skills
- Well-developed IT skills with the ability to navigate client management systems.
- Ability to work within and positively advance the mission and values of Green Heart Care.
- Knowledge and experience of the Support Coordinator NDIS function and why it is important in building a Participants social capacity and support the success of our funded programs.
- Previous experience working within a human services environment such as - Aged Care, Disability Care, Mental Health, Housing or a complimentary sector.
