Bodewell is seeking a highly skilled and experienced Clinical Care Consultant to provide exceptional person-centered care management and clinical assessment for home care clients in VIC. Reporting to the Home Care Manager, you will play a dynamic role in ensuring the safe provision of Clinical Care, compliance with regulations, and building positive relationships with clients and stakeholders.

About the Role
As a Clinical Care Consultant, you will manage a caseload of up to 15 clients, coordinating in-house nursing services and proactively reclaiming outsourced nursing hours. You will utilize your robust understanding of Home Care & Clinical Compliance and knowledge of Home Care Quality Standards to deliver specialized care in areas such as wound management, continence management, medication management, dementia care support, and diabetes management and education.
Key Responsibilities Include:
- Managing a caseload of up to 15 clients, ensuring exceptional care and well-being.
- Coordinating in-house nursing services and reclaiming outsourced nursing hours proactively.
- Conducting clinical assessments and interventions in specialized areas.
- Demonstrating comprehensive knowledge of industry regulations and best practices.
- Maintaining compliance with legislation, guidelines, and standards.
- Building and maintaining positive relationships with clients, families, and stakeholders.
- Mentoring and supporting direct care staff.
- Participating in quality reporting activities and advocating for consumers.

Qualifications:
- Hold and maintain current AHPRA RN registration with the Nursing and Midwifery Board of Australia.
- Tertiary qualification in Nursing.
- NDIS Worker Screening Check.
- Right to Work in Australia.
- WWCC
- Current State Driver’s License with reliable vehicle.
- Computer literacy - including Microsoft Office, Internet and Database Systems, CL+.
- Current required vaccinations.
- Knowledge of the Home Care Package Program.
- Experience in Community Nursing.
