About the Role

An exciting opportunity currently exists for a Grade 3Social Worker on a permanent, part time basis (24 hrs per week). You will work within a multidisciplinary team who delivers excellent patient centred care and lead on ensuring the clients are linked into services and community supports.

As the CHSP Social Worker in CDAMS-GMC, you will work closely with clients presenting with frailty and/ or cognitive impairment who require assistance to connect with support services. The aim of the role is to enable the clients to maintain their quality of life and remain at home as long as possible and support their carer(s) to help minimise carers strain.  

This will include supporting the client and carer with linking to community services such as Dementia Australia, Dementia Support Australia, community groups, My Aged Care, NDIS, Legal services, social groups, carer support services and requires an excellent knowledge of the same. The interventions are generally short term (approx. up to 3 months).

Classifications for this position will range from SC31 to SC34 ($102,091 - $116,646 per annum, pro rata for part time)

Your Contribution

  • Bachelor or Qualifying Masters of Social Work

  • Minimum of seven (7) years’ post-graduate experience

  • Extensive experience working with clients over the age of 65 with multiple health issues, comorbidities, neurocognitive disorders and complex care needs.

  • Extensive experience working with clients/carers in relation to capacity, VCAT and legal pathways.

  • Highly developed leadership and interpersonal skills that incorporate excellent communication.

  • Demonstrated commitment to service improvement and professional development.

  • Capacity to work collaboratively with a wide range of health professionals and external stakeholders to facilitate the achievement of health and well-being outcomes.

  • Excellent organisational and time management skills.

  • Well-developed computer skills (outlook, word, excel, patient management systems)

  • Current national criminal history check, or willing to obtain

About Department

This position is part of the Home and Community Care Program for Younger People (HACC-PYP) and Commonwealth Home Support Program (CHSP) within St Vincent’s Hospital, Melbourne, which also includes nursing, and allied health, and will be co-located with the Cognitive Dementia and Memory Service (CDAMS) and Geriatric Medical Clinic (GMC) based at St George’s Hospital in Kew.

The role provides direct service delivery, and tailored support to those experiencing diverse barriers to accessing services, assists linking clients and carers in with appropriate services, education, and community supports, to enhance their quality of life.
