Clinical Care Coordinator

Meracare Aged Care is currently looking for a Full-Time Clinical Care Coordinator to oversee clinical services at our family owned home in Wangaratta. Meracare Wangaratta is a 60 bed facility where we work with our residents to provide a person-centered care approach and current best practice strategies to enhance the lives of all the residents.

The Clinical Care Coordinator will provide clinical leadership and work collaboratively with the Facility Manager and key relevant people, residents, and families to oversee and guide effective resident focused clinical outcomes. 
 

The successful candidates are required to:

• Use your strong clinical skills to provide high quality standards of care
• Provide leadership, guidance and oversight of all clinical services; demonstrated ability to lead a clinical team
• Provide assessment, planning, implementation and evaluation for resident care 

You must:

• Hold current registration with Australian Health Practitioners Agency (AHPRA)
• Be experienced as an Enrolled Nurse or Registered Nurse in Aged Care with strong clinical skills
• Be experienced in a residential care setting
• Have a passion for making a difference in the lives of aged individuals
• Have an understanding of AN-ACC funding model
• Have an understanding of the Aged Care Quality Standards

Please note that Meracare require the successful applicant to hold a satisfactory police clearance and NDIS workers screening check prior to an offer of employment.
