Clinical Care Coordinator (Registered Nurse G5)

A rewarding opportunity at our Avondale Heights Residential Aged Care Home for a Clinical Care Coordinator. This is a full time position with the following roster:

Monday, Tuesday, Wednesday  0700-1506 and Saturday, Sunday 0900-1706.

The role will be responsible for the delivery of Clinical Care services including care planning, review and assessments. 

What you will bring:

• Dedication to resident care and support
• Excellent clinical knowledge regarding medical conditions, palliative care, and care of people living with dementia including managing challenging behaviours
• High level interpersonal and written skills
• Experience managing staff and performance management
• Ability to effectively communicate with residents, resident representatives and other health professionals
• A good understanding of the aged care reform agenda, including AN-ACC funding and royal commission recommendations related to Clinical Care
• Current AHPRA Registered Nurse DIV 1 registration with no conditions
