What your role will look like
Reporting to the Access Coordinator, we are seeking an experienced Clinical Care Coordinator to work in a Part-Time basis in Melbourne.
The successful candidate will work alongside the Quality Compliance and Risk Manager, Learning and Development Coordinator and will lead, support, and educate the staff within the home, maintaining clinical excellence in the provision of quality care.
Responsibilities include:
- To co-ordinate the Clinical Care needs for clients, ensuring the client needs are met and regularly reviewed.
- Manage all clinical services in line with the company's policies and processes.
- Assist with the development and maintenance of clinical documentation, assessment, and care planning for residents.
- Work alongside a Access Coordinator to provide direction and supervision to the care staff.
- Oversee the Medication Management Policy.
- Co-ordinate and document Case Conferences and Multi-disciplinary team meetings to review and evaluate client care.
- Supporting the Access Coordinator to ensure that the quality documentation are compliant at all times.
- Timely follow-up of client clinical issues; hazards; incident reports; and infection control issue
To be successful in this role - Previous senior Clinical Care experience in the health sector with clinical experience in managing clients with complex health issues, specialising in ventilation and tracheostomy management
- You are a Registered Nurse with AHPRA
- NDIS Workers Screening Check
- You have a sound knowledge of NDIS and preferably have worked with NDIS clients previously
- You are flexible, energetic, innovative, and can work cooperatively in a multidisciplinary team as well as autonomously
- Demonstrated organisational skills and ability to prioritise work whilst maintaining a commitment to quality of care and best practice
