Are you an Enrolled Nurse who can match our energy and drive to truly make a difference. You will understand every resident is someone’s mother, father, sibling or spouse and your responsibility as Clinical Care Funding Assistant puts you in the front line of creating a better future for them by ensuring we are appropriately financially resourced.
Reporting to the Clinical Governance Manager, you will deliver a disciplined approach to the implementation of the Aged Care Funding Instrument (ACFI) to achieve Palm Lake Care financial KPIs by appropriately balancing care needs and resident choice. To be successful requires active listening and ability to work as part of a team and autonomously. You must be a passionate and effective educator able to motivate and guide the behavior of busy colleagues. You will do this with understanding underpinned by an unwavering commitment to drive documentation outcomes.
Your role.
As the Clinical Care Funding Assistant, you will be primarily responsible for ensuring all residents have relevant charting and completed on time, every time. You will support admission and review process and attend all validations at the community and when directed at other PLC communities. You will communicate with clinical and care colleagues, management, and residents on an ongoing basis to ensure quality documentation outcomes.
The dignity and integrity of each resident’s independence will be based on your comprehensive and accurate nursing assessments, and your engagement in nursing practices, allowing you to understand and see the positive changes firsthand.
A few obvious things.
- Diploma of Nursing with current AHPRA registration
- Minimum 3 years’ experience
- Experience working in an Australian Aged Care Facility
- Demonstrated commitment to continuous improvement
- You will be team orientated to achieve the best possible outcomes for our residents
- Excellent people and communication skills
- Empathy with elderly residents and their families
- Knowledge of ACFI and Accreditation standards would be advantageous
- Knowledge of Autumn Care would be highly regarded
- Experience in commissioning an aged care facility would be advantageous
- National Police Check
