About our Home Menora Gardens is in the suburb of Menora WA, approximately 7 km from the Perth City Centre.Menora Gardens is home to 178 residents (includes a Secure Memory Support Unit for 50 residents) it is a modern building with landscaped gardens and outdoor areas for residents to enjoy, with quality services and catering. About the Opportunity We are seeking an experienced Clinical Coordinator to join our team working on a permanent part-time basis on Night Duty. Reporting to our General Manager you will be responsible for ensuring consistently high standards of care to our residents, to “make every day the best it can be” whatever their capability. Your responsibilities are to ensure that residents receive the prescribed personalised care. Liaise with the multi-disciplinary team to minimise falls, wounds, etc. providing data and reporting to the Home Leadership Team and Clinical Governance team. Your role will include, but are not limited to: Prescribing and ensuring the delivery of personalised care to all residents, meeting the Aged Care Standards. Ensuring staff are compliant Bolton Clarke policies and procedures. Developing, implementing, and monitoring resident assessments and care plans Assisting with the data collection for monthly KPI’s and NQIP reporting Performance Management of your team Contributing to the Quality Improvement Plan and implementing agreed clinical improvements. Actively supervising, training, and mentoring staff on duty as needed Build a functioning team of care staff to enable the above About You To be successful in this role you will have. Current RN, registration with AHPRA Minimum of 3 years post-registration experience as an RN Knowledge of the aged care standards and changes to these going forward Sound understanding of the accreditation process. Excellent communication, interpersonal and leadership skills Experience in performing comprehensive nursing assessments. Experience investigating and reporting resident issues and incidents.
