About the Role

This is an exciting opportunity for a Clinical Coordinator (Registered Nurseto join our Street Doctor street-side mobile medical service. Black Swan Health’s Street Doctor aims to improve health outcomes for people experiencing disadvantage and marginalisation. Our RACGP accredited Street Doctor service is provided by General Practitioners, supported by Nurse Practitioners, Registered Nurses and Outreach Workers, who offer people the highest quality health care. 

Black Swan Health’s Street Doctor clinics are delivered in diverse and interesting locations across the north and south metropolitan areas.

This is an integral role in a dedicated, passionate and skilled working team whose main focus is ensuring all people are able to access the highest quality medical care. 

The Clinical Coordinator works closely with the Service Coordinator to ensure a broad range of clinical support is available to General Practitioners and Nurses in the Street Doctor mobile medical team. 

The successful applicant will oversee infection control activity, stock control, medication and emergency management process compliance and maintenance of the cold chain systems and processes. The Clinical Coordinator will undertake patient recall, reminder and follow up activities and may undertake clinical duties on-site as part of the mobile clinics. 

This position will support the review and development of clinical based policies and procedures in keeping with an accredited general practice.

The successful candidate will 

  • Possess the relevant Tertiary qualification (Registered Nurse) and hold membership to the appropriate professional body (AHPRA).
  • Provide clinical support to the Street Doctor team including Service Coordinator, GPs, and Registered Nurses 
  • Manage medical supplies and medication resources, including safe disposal of expired medications
  • Lead cold chain management practices
  • Provide clinical advice to the Service Coordinator to support effective clinic delivery
  • Complete patient follow up as requested by GP’s (including recalls and reminders) appropriately and efficiently
  • Demonstrate experience in primary health care
  • Have excellent communication, organisational and time management skills 
  • Support the Service Coordinator in accreditation and reaccreditation processes to develop guidelines and protocols and use the best practice guidelines to inform clinical care management
  • Current National Police Clearance
  • Current Working with Children's Check
  • Current eligibility to work in Australia, with no restrictions.
