As a Clinical Home Support Assessor with Aspire4Life, you will be at the forefront of ensuring that older Australians receive the supports they need to maintain their independence and well-being. You will be utilising the My Aged Care assessment framework which places a strong emphasis on promoting wellness and reablement. You will travel throughout the Far North Coast Aged Care Planning Region to collaborate with older Australians to develop personalised support plans that align with their unique life stories and aspirations.
Aspire4Life offer comprehensive training and orientation, held both virtually and/or face-to -face in our Coolangatta office (all expenses covered).
This position will commence on a fixed term contract until 30 June 2024 (with possible extension based on funding).

A typical week as an assessor with Aspire4Life:
- Conduct in-home client-centred assessments
- Create individualised support plans aligned with clients goals
- Recommend and refer clients to appropriate services
- Foster collaboration with clients, their loved ones and community supports
- Maintain accurate documentation and reporting
About you:
- Local knowledge and the ability to network with stakeholders within your region
- Confident communicator with the ability to build trust and rapport
- IT skills with previous experience working across Microsoft Office Suite
- Previous experience working with KPI's
- The ability to manage your time effectively in an autonomous environment
What to bring:
- Tertiary qualified Registered Nurse, Occupational Therapist, Physiotherapist with current AHPRA Registration or Social Worker with AASW Registration
- Unrestricted Australian driver's licence
