Position Profile and Role:

We are excited to announce new opportunities within our growing Safety & Quality team, focusing on operational safety across clinical functions, recognising the different operational settings, nature of care being delivered, and spectrum of patient needs our services respond to.

We are seeking Clinical Incident Management Coordinators for two roles:

  1. Clinical Incident Management Coordinator - Perth Children's Hospital and Neonatology.
  2. Clinical Incident Management Coordinator - CAHS.

Reporting to the respective Directors of Safety, Quality & Performance, both roles manage Clinical Incident and risk management programs, with a focus on maintaining and enhancing patient safety and ensuring compliance with national standards.

You will provide expert advice on clinical risk management and work with directors and department heads to implement and evaluate mitigation strategies, supporting the organisation to effectively manage and learn from Clinical Incidents

In parallel you will educate and support staff in Clinical Incident awareness, investigation methods, and risk management principles as a key activity in embedding the CAHS clinical risk management system.

Key responsibilities for both roles include:

  • Overseeing the CAHS Clinical Incident Management system, including severity assessment, notification, and investigation of serious adverse events (SAC 1 and high-risk SAC 2).
  • Guiding incident response, triage, on-the-ground investigations, and analyse Clinical Incident data to identify risk minimisation strategies.
  • Providing expertise in Clinical Incident investigation methods.
  • Liaising with stakeholders to review significant incidents and disseminate lessons learned.
  • Implementing and maintaining the CAHS Open Disclosure framework, facilitating open disclosure conversations.
  • Coordinating external reporting to regulatory bodies.
  • Monitoring, reviewing, and reporting on clinical risk management performance to governance committees and stakeholders.
  • Collaborating with the Safety Quality Team on national standards and accreditation processes, ensuring open communication and promoting a culture of innovation and excellence.

The ideal candidate will have extensive experience in Clinical Incident investigation methods and risk management principles, with strong analytical skills and the ability to interpret incident data and identify trends.

Clinical Incident Management Coordinator - Perth Children's Hospital & Neonatology

You will lead Clinical Incident and risk management programs across Perth Children’s Hospital and Neonatology (PCHN). This role focuses on safeguarding patient safety and ensuring compliance and operational continuity within our acute care service. 

  • You will manage the CAHS Clinical Incident and risk register from a clinical perspective across patient care, clinical operational continuity, and healthcare interventions.
  • You will lead implementation of the Department of Health’s Clinical Incident Management Policy, and work in liaison with PCHN, coordinating and facilitating open disclosure conversations as part of Incident Management.
  • As an integral part of the PCHN Safety & Quality Team, you will collaborate with the team to maintain compliance with national standards and accreditation processes, and disseminating lessons learned locally.

Clinical Incident Management Coordinator - Child & Adolescent Health Service

You will lead Clinical Incident and risk management programs across the Child & Adolescent Health Service (CAHS) ensuring compliance with national standards and enhancing patient safety across multiple sites. This role focuses on system-wide strategic oversight across CAHS, drawing on analysis to provide recommendations and drive quality improvement.

  • With oversight of key performance indicators, you will monitor SAC1 incidents, reporting up to the Safety & Quality PPEC (performance & Planning Executive Committee meeting).
  • You will be responsible for facilitating SAC1 reviews over multiple sites as required, and work closely with stakeholders and clinical departments to build their awareness of effective governance as part of our continuous effort to minimise risk, improve safety and elevate quality of care.
  • You will liaise with the audit and risk team regarding the CAHS Clinical Incident register and risk register, and disseminate lessons learned globally.
