Core Role Responsibilities
Care Consults :
- Attend Caring Consults, assess Clients with respect to physical, emotional and environmental needs and how we can make a difference.
Client Care Delivery and Experience :
- Ensure an appropriate and individualised care plan is developed before the commencement of care programs.
- Work in collaboration with leadership team to review clinical skills and experience of team members, ensuring team members are capable and confident in the delivery of support to assigned clients.
- Conducts supervisory visits to ensure Caregiver fit and quality of care; delivering a premium client experience.
- Monitor and review team member shift visit notes ensuring changes in client conditions are appropriately addressed.
Community Nursing :
- Attend to community nursing supports - wound management; catheter management, medication administration & management.
Administration :
- Maintain excellent documentation practices - specific focus on accurate care plans.
- Degree Qualified - Nursing
- Minimum 5 years nursing experience (community highly desired)
- Leadership experience desired
- Nursing administration experience desired
- Capacity to drive between southern Gold Coast and Lismore
