About Us

BelleJoy provides 24/7 care to NDIS Participants in Supported Independent Living accommodations and the Community. All homes feel like home as we offer top-of-the-range furnishings, housing and equipment as we believe a comfortable environment for our Participants is crucial in the services and care we offer. Our participants are encouraged to develop social skills and increase their capacity to be independent at home and in the Community. 

The Clinical Care Team Leader Support Independent Living (SIL) brings a best-practice Clinical Care focus to the organisation and our teams. They are also responsible for providing participants with support that is in line with their goals and NDIS SIL plan as well as assisting them to live independently in their place of residence. The Clinical Care Team Leader also provides Team Leadership and support, employee supervision, direction for employees on a day-to-day basis, oversight of the SIL property operations and develops staffing rosters. Manage costings and allocated budget for the house. They are expected to provide information on program services and community supports as required, actively engage in professional supervision, individual work programming performance planning and professional development opportunities, and work collaboratively with colleagues and management. 

This role will suit a Registered Nurse who is APHRA registered, with a proven track record in leading, managing and developing/training staff.

RESPONSIBILITIES AND DUTIES

  • Lead the team and work as an individual practitioner.
  • Develop, monitor, coach and train for individual and team outcomes and performance.
  • Lead the training components across our teams who are delivering high-intensity clinical support to our complex high-intensity Clinical Care participants.
  • Share operational or procedural direction and advice with your team.
  • Seek advice on complex issues where some liaison with other teams or organisations is involved.
  • Prepare monthly management reports.
  • Ensure participants are appropriately supported to resolve concerns or issues.
  • Ensure quality of service across the team.
  • Manage the safety of the team through thorough induction, OH&S, Risk Assessments and supervision.
  • Ensure team members possess the required certification and check with the HR Manager.
  • Have input to and ensure adherence to organisational policies and procedures and all relevant government legislation and standards.
  • Ensure the service provision is in line with the NDIS practice standards and NDIS Code of Conduct.
  • Using assessments, allied health, and clinical recommendations to ensure participants are receiving care that supports their individual needs. 
  • Engage in the development, implementation and evaluation of services and programs that meet the participants' needs.    
  • Ensure the provided service is directed by the participant's individual needs and is in line with current protocols or individual Care plans from BelleJoy or other services. 
  • Maintain professional relationships with families and carers and acknowledge their input as part of the participants' support team. 
  • Meet compliance with all relevant legislation and service delivery standards. 
  • Be compliant with all company policies and procedures.
  • Support clients to access activities and outings within their Community.
  • Ensure best practice is delivered by with the medication administration policy, work health and safety procedures and client rights policies including privacy and confidentiality.
  • Ensure completion of appropriate documentation. Relevant information on support provided is to be recorded in client notes and communication books as required.
  • Ensure each Participant receives safe and effective services and supports for daily living that are culturally safe and meet the participants' needs, goals and preferences and optimise their independence, health, well-being, and quality of life.  

DIRECT SUPPORTS

  • Provide personal care including prompting and full assistance where necessary, for showering and all aspects of personal hygiene care.
  • Provide mobility support as directed by allied health, to ensure the safe transfer and mobility of participants.
  • Support participants with skill building in domestic duties to maintain their homes and gardens.  
  • Support participants with meal planning, grocery shopping and meal preparations. 
  • Encourage inclusion in community events including mainstream events. 
  • Where possible upskill and encourage independence with Travel training.
  • Support to maintain meaningful employment when identified and implemented by the participant or their Coordinator of support.  
  • Ensure participant records and progress notes are completed thoroughly on every shift.
  • Support participants with developing their budgets so they can work towards identified goals. 
  • Prompt and monitor safe medication administration. 
  • Support staff are required to have a thorough knowledge of an individual participant, to regularly review participant documents to ensure they are up to date and to support the person to appointments. 

STAFF/TEAM

  • Ensure the successful implementation of the Strategic Plan with your team.
  • Coordinate all aspects of the service including relevant documentation completed, the daily activities of participants and staff and the development of rosters to ensure that all shifts are adequately filled.
  • Able to train staff in manual handling, hoists, bowel and catheter care.
  • Provide leadership to the service and direct reports, creating an environment that promotes reflective practice and builds the capacity of the team. Oversee performance and development planning, managing performance issues that may arise as per policy and procedure to optimise team performance.
  • Build and maintain a current working knowledge of local community supports and services to ensure a holistic person-centred approach to service this includes contributing to key community and interagency meetings/forums, advocating, and raising awareness of key issues.
  • Provide support to people with disabilities in areas such as personal care, health and wellbeing, accessing the local community, learning Independent Living skills and implementation of all supports by their funded support packages.
  • Communicate effectively with all stakeholders in a professional manner and ensure ongoing positive relationships are formed with funders, peak bodies and other key stakeholders.
  • Oversee the coordination of data collection within the service, including reviewing daily, and ensuring compliance with current legislative and NDIS registration requirements and organisational processes.
  • Participate in training to assist the team in maintaining optimum service delivery for participants.
  • Develop and maintain a positive team environment.
  • Communicate with the Team Leaders to ensure a consistent and seamless approach to service provision.

WORK HEALTH AND SAFETY

  • Ensure all manual handling, infection control, medication administration and other safe working procedures are always followed. 
  • Report all injuries, hazards, and incidents to a Team Leader promptly on the appropriate form. 
  • Carry out all duties with diligence and due care for personal safety and the safety of others. 
  • Comply with reasonable instruction, policy or procedures given or notified by the NDIA relating to health or safety across the NDIA.

ACCOUNTABILITY

  • The Clinical Care Team Leader reports directly to the Program Manager/General Manager.

SECURITY

•           NDIS Worker Screening Check

•           Current NSW Driver’s Licence

•           Must be an Australian Citizen and have working rights in Australia.
