The opportunity
Silverchain are currently looking for suitably experienced Clinical Nurse (Case Coordinator) to join our Country Health Service team in Bunbury on a permanent Part-time basis (5 days per fortnight), you will work alongside the other Case Coordinators on a rotational basis across 7 days Monday – Sunday, 8.00am – 4.00pm.
Key Responsibilities
Become part of a dedicated and passionate team, work with your Clinical Nurse Manager and other Case Coordinators to ensure effective delivery of clinical services. You will be accountable for the safe delivery of quality evidenced based clinical practice to the clients in your area, and responsible for the competency and performance of clinical staff in your team.

About you
You will be committed to providing a quality service and excellent attention to detail. You will also meet the following criteria:
- Current Registered Nurse registration with the Australian Health Practitioner Regulation Agency
- Demonstrated experience in leading a team and coordinating resources.
- A minimum of 5 years’ experience as a Registered Nurse
- Ability to make informed decision and solve problems.
- A strong team focus with the ability to also work autonomously.
- An understanding of care delivery requirements within a community environment
- A current Australian driver’s licence
