The purpose of the Clinical Nurse (CN) – TCHS is to promote and maintain the health and wellbeing of clients within the community setting and acute to community interface by utilising holistic and comprehensive assessment practices to formulate and implement care plans in consultation with the client and key internal and external stakeholders.
This role practices in all areas of TCHS: Discharge Coordination, Post-Acute Nursing Care, Community Assessment and Case Management, Post-Acute Wound Clinic, Intake and Community Intravenous Program, Refugee Health.
Role requirements:
- Minimum 2 years Community Health nursing or transferable skills.
- Broad acute and post acute nursing experience.
- Proven ability to comprehensively assess, plan, implement and evaluate nursing care at an advanced level within a complex environment.
- Demonstrated ability to build and maintain effective relationships with team members, patients and other stakeholders, including participation in relevant networks to achieve work unit outcomes.
- Ability to provide nursing leadership in the clinical and professional setting including the role modelling of effective communication and problem solving.
- Proven ability to use information and reporting systems appropriate to the position in order to achieve outcomes and identify areas for improvement.
- Proven ability to provide clinical education and support at an advanced level and demonstrate own professional development.
Generous remuneration package value up to $128,435 p.a. comprising:
- Salary range of $104,816 to $112,236 p.a. (full-time equivalent) (Nurse Grade 6 Band 1)
- Salary range of $52.86 to $56.60 p.h. (part-time equivalent) (Nurse Grade 6 Band 1)
- Annual leave with 17.5% loading
- 12.75% employer contribution to superannuation.

Requirements
- Bachelor of Nursing or equivalent certification relevant to the position is mandatory.
- Proof of qualification, registration with the Australian Health Practitioner Regulation Agency (AHPRA) and possession of a current practising certificate.
- This position requires the incumbent to operate a class C motor vehicle and an appropriate license endorsement to operate this vehicle is required.
- This is a Vaccine Preventable Diseases (VPD) risk role: Hepatitis A/B, Measles, Mumps, Rubella, Varicella, and Pertussis.
