What you'll be doing
This position provides a key nursing leadership role in the advanced and complex Care Coordination for at risk patients in conjunction with the multidisciplinary team at Blacktown Cancer and Haematology Centre and across the Sydney West Cancer Network as appropriate.
The CNC’s role is to ensure seamless continuum of care between the hospital, the Sydney West Cancer Network and the community. As well as acting as resource between patient and hospital in prevention of preventable complications of disease and hospital admission reduction where clinically possible
