This is an exciting new position providing support to people presenting to Emergency Departments across Northern Sydney Local Health District with complex chronic health and social care needs. The aim of the program is to provide a whole of person care approach tailored to the individuals needs to address the factors driving frequent Emergency Department use.

Where you'll be working
The role is based at Macquarie Hospital, North Ryde; however, the interventions will be provided in the community across NSLHD, in clients homes with in-reach to all four Emergency Departments in NSLHD (Royal North Shore Hospital, Ryde Hospital, Hornsby Hospital and Northern Beaches Hospital).
What you'll be doing
The Emergency Department to Community (EDC) is a program that works closely with patients who frequently present to Emergency Departments across Northern Sydney Local Health District (NSLHD). EDC provides tailored intensive case management and specialist care to clients in the community, improving their health and reducing the need for Emergency Department presentation. The service delivery model will include provision of community care in clinics and client homes, and in reach to all four Emergency Departments in NSLHD (Royal North Shore Hospital, Ryde Hospital, Hornsby Hospital and Northern Beaches Hospital).

Candidates will need to meet the following criteria- Registered with Authority to Practice with Australian Health Practitioner Regulation Agency. Minimum seven years full time post graduate experience with 5 years full time equivalent in the specialty field. Specialty field relevant to the EDC program includes mental health, drug and alcohol, pain service and/or community health. In addition, the applicant must have an approved post graduate qualification (minimum Grad Diploma/Certificate level) or evidence of working towards Masters’ level qualification.
- What experience do you have working with clients with complex health and social needs, particularly those with mental health, drug and alcohol and pain, based in the community.
- What experience do you have working with varied stakeholders including health and non government organisations to plan, coordinate and evaluate plans of care to address complex health and social needs.
- What experience do you have in providing leadership, performance monitoring and evaluation for a program.
