An exciting temporary full-time position exists for a highly experienced and expert nurse within the Geriatric Rapid Acute Care Evaluation (GRACE) team at Hornsby Ku-ring-gai Hospital (HKH). The position is required to provide a high-quality outreach service, providing care at home, and aiming to avoid hospital admission for people who reside within the community in a private residence or within residential aged care facilities. The position works closely with the multi-disciplinary team, comprising of Geriatrician, Nursing (Clinical Nurse Consultant, Clinical Nurse Specialist, Registered Nurse), Allied Health Physiotherapist and Administration.

The GRACE service at HKH operates from Monday to Friday 8am to 8pm, the successful applicant will be required to work rotating shifts of 8- or 10-hours duration.
What you'll be doing
The NSLHD Nursing and Midwifery Workforce will be compassionate, competent, confident, contemporary, capable and committed to leading person-centred health care.
The Clinical Nurse Consultant (CNC) is an expert nursing who will provide leadership, advice and recommendations on all aspects of nursing and consults on matters relating to the safe and appropriate use of nursing practices.
The CNC will formulate and implement policy, guide practice development, and engage in education and research activities. The CNC is responsible for the provision of clinical supervision to nursing staff and acts as a role model, providing nursing care on a regular basis.
- To provide assessment and treatment for older people living in the community who become acutely unwell and are at risk of hospitalisation, as part of the Geriatric Rapid Aged Care Evaluation (GRACE).
- To provide practical support for Residential Aged Care Facilities in managing COVID -19 outbreaks.
- To develop education and training regarding COVID -19 outbreaks in Residential Aged Care Facilities.
- Exercise extended autonomy of decision making.
- Exercise professional knowledge and judgement in providing complex care requiring advanced clinical skills.
Candidates will need to meet the following criteria:
- Registered with Authority to Practice with Australian Health Practitioner Regulation Agency. Minimum five years full time post graduate experience with 3 years full time equivalent in the specialty field. In addition the applicant must have an approved post graduate qualification (minimum Grad Diploma/Certificate level) or evidence of working towards Masters level qualification.
- Demonstrated clinical leadership skills and an ability to work collaboratively within a multidisciplinary framework with a demonstrated commitment to excellence in nursing practice.
- Demonstrated ability to work effectively both as a member of a multidisciplinary team and independently.
- Current unencumbered NSW Driver's Licence.
