About the role
The purpose of this role is to provide advanced clinical expertise and care relevant to patients within the Intensive Care Unit (ICU). The role will primarily support coordination of care for patients and facilitate referrals to the Follow Up Clinic which provides support to patients who have had an ICU admission. This role will work closely with the ICU multidisciplinary team to coordinate and manage the care of patients accessing this service.
