About the role
The Clinical Nurse Consultant (CNC) will provide expertise and leadership for ensuring the Residential Aged Care Acute Support Service (RaSS) & Geriatric Emergency Department Intervention (GEDI) across West Moreton Health (WMH), is evidence based and of the highest quality, meeting relevant standards and performance indicators. The CNC provides an advanced practice role with a multidisciplinary approach to the screening, assessment, care coordination and care planning of the elderly (generally aged over 70 years) who present to the emergency department. The RaSS/GEDI team work collaboratively with the older person, their caregivers, and the broader healthcare team. The CNC provides nursing leadership to the multidisciplinary team in order to facilitate the best outcomes for the older person presenting to the department.
The Clinical Nurse Consultant:
- has an expert level of clinical and professional practice;
- has an advanced level of decision-making skill, knowledge and capability;
- works collectively to apply the principles of good clinical governance; and
- applies professional and clinical expertise in collaboration with nursing and other stakeholders.
About you
Our ideal candidate will be someone who can demonstrate the following:
- Demonstrated advanced understanding, clinical evidence-based skill and knowledge in Older Person Care and will undertake leadership for enhanced delivery of care for older persons in the Emergency Department.
- Demonstrated leadership and management capability through the expressed knowledge of team engagement and human, financial and material resource management principles applicable to the planning, coordination and evaluation of services.
- Demonstrated highly developed organisational skills and advanced written and oral communication that influence effectively and achieve desired outcomes, including the ability to establish and manage priorities, coordinate team activities in alignment with service priorities and respond effectively to emergent issues to assure effective service delivery.
- Demonstrated ability to use clinical information and other relevant information technology systems to support decision making, conduct surveillance, perform activity reporting and facilitate quality improvement strategies.
- Demonstrated ability to work independently whilst operating effectively in a diversified team.
Mandatory Requirements:
- Vaccinated against, and remain vaccinated against, certain vaccine preventable diseases (VPD) as outlined in the Health Employment Directive NO. 01/16
