About the Role
- Take a leadership role in liaising with stakeholders in the ED and across community mental health teams including the Transitions Coordinator to assist in streamlining consumer flow across various mental health settings including ED, community and inpatient services through proactive care planning.
- Work collaboratively with consumers and stakeholders on acute management plans, safety plans and relapse prevention plans to streamline assessment and intervention for consumers with complex mental health concerns presenting to the ED
- Participate in the strategic development of a work-based culture that promotes contemporary evidence-based practice through education, learning, research and workforce development.
- Role model and lead the provision of safe quality care and practice in meeting the National Safety and Quality Healthcare Standards and other standards for healthcare including benchmarking.
