Continued Care Services offers intervention and case management for consumers requiring medium to long term support.This role is required to deliver direct clinical care at an advanced level, to consumers requiring mental health interventions in the rural and remote communities within the NWHHS, whilst providing support and education to other team members.This will entail the provision of specialist community services that promote the health and wellbeing of consumers in these communities, within an integrated recovery focussed model of care incorporating hospital and community components.Travel within the NWHHS may be required to provide outreach services.

Permanent full-time role, 76 hrs p.f.Base salary range between $107,960 and $115,604 p.a. (Nurse Grade 6 - Band 1)
Requirements:- Proof of qualification, registration with AHPRA and a current practising certificate are mandatory requirements.
- This is a Vaccine Preventable Diseases (VPD) risk role and may require evidence of vaccination or immunity to: Hepatitis B, Measles, Mumps, Rubella, Varicella, Pertussis and Tuberculosis.
- A 6-month period of probation may apply.
