• Lead and provide expertise in community care for older people.
• Educate and empower nurses to enhance their practice.
• Foster partnerships with stakeholders through participation in professional groups.
• Resolve complex client situations to ensure safety and uphold reputation.
• Develop and deliver education for junior staff.
• Deliver clinical nursing care and support.
• Coordinate nursing assessments and reviews.
• Collaborate on admissions and discharge summaries.
• Provide clinical input and assist in health assessments.
• Case manage complex clients with high needs.

Skills & Experience:

• Minimum two years as a Clinical Nurse.
• Three years as a Registered Nurse.
• Recent community care experience.
• Experience in holistic chronic condition management.
• Provided mentorship in clinical settings.
• Focus on continuous improvement.
• Proficient in dementia care.
• End-of-life care expertise.
• Adaptability in changing care needs.
