The purpose of the Acute Care Team CNS 2 position is broadly defined as:
Providing consistent clinical leadership and demonstrated decision making across the team.
Proactively supporting a least restrictive model of care through actively managing clients in the community
Working in collaboration with the senior management team and ACT clinicians in developing and supporting best clinical practices associated with acute care coordination services and optimal outcomes.
Exercising professional knowledge and judgement in providing and supporting the use of a range of evidenced based psychosocial treatment interventions.
Liaising and working collaboratively with the ACT Crisis Team when required around includes assessment, collaborative planning, intervention and evaluation of care to people who experience enduring and complex mental health problems.
People of Aboriginal and/or Torres Strait Islander background encouraged to apply.Candidates will need to meet the following criteria:
A Registered Nurse with relevant post registration qualifications and at least 3 years’ experience working in the clinical area of their specified post-graduate qualifications or such other qualifications or relevant equivalent experience.
Demonstrable experience of working collaboratively with consumers to identify and work towards meeting their goals from a recovery oriented practice framework
Demonstrated high level interpersonal and communication skills including advocacy and negotiation and the ability to work effectively within a multidisciplinary team
Experience working collaboratively and in partnership with key stakeholders including knowledge of community supports
Demonstrated experience in initiating, developing and leading activities to improve delivery of clinical services
Demonstrated clinical leadership skills, commitment to consumer focused care, and experience in providing complex care in the area of mental health nursing including care coordination
Current NSW driver’s license with the ability to drive across the geographic location of Northern Sydney Local Health District
