LDK (Love, Decency & Kindness) Seniors' Living was established by our Founder and Director, Paul Browne, who felt called to the industry 20 years ago in response to personal experiences with senior loved ones.
Paul saw there was a stark difference between the vibrant lifestyle experienced in a retirement village versus the institutionalised environment of a nursing home. As retirement village residents’ care needs increased, they often had to move out of their village home into a nursing home, losing the lifestyle, privacy and purpose they experienced in a retirement village. Paul sought to bring together the vibrancy of a retirement village with the care capabilities residents require in their ageing journey to create a revolutionary integrated offering providing vibrancy and care in one location.

As the Clinical Case Manager, your role will be absolutely vital in delivering exceptional and holistic care to our valued residents. With your expertise, you'll take charge of clinical assessments, crafting comprehensive care plans, and orchestrating the coordination of all necessary support services. As a prominent leader within our dedicated care team, you'll serve as a guiding force, empowering every team member to truly make a meaningful impact on the lives of our residents each and every day. What You’ll Bring to the Team:To be considered for this role, you must hold (at a minimum) a Bachelor’s Degree in Nursing and be registered with AHPRA. You will also need to have demonstrated experience in the community care sector and/or home care package management, with prior nursing experience in a senior’s living environment.
