We are a private organisation and a leading national team providing in-home Aged Care to seniors with the highest standards of care. We are currently seeking outstanding and passionate Clinical Care Manager on a Full-time basis.
The Clinical Care Manager will provide a dual role managing a case load of their own clients and also providing clinical assistance to the team. They will be a key member of the services team providing assessment, care planning, monitoring and referrals for all clients. The Care manager is responsible for performing in home face to face assessments and partner with clients, their carers/families to assess current situations and create case plans that enable maximum independence and positive impact for the person. Care managers also work creatively and resourcefully with participants on how they utilise their support budgets to achieve their goals.
Main responsibilities of Care Manager:
1. Person centered assessment and care planning
• In consultation with clients, carers and families facilitate care assessment, care planning and review
• Develop and monitor individual plans and budgets with regular reviews/case conferencing
• Manage any identified risks within principles of Positive Approach to Challenging Behaviours and Duty of Care, Dignity of Risk
• Development and maintenance of all documentation in client management systems
• Work within legislated CDC frameworks
• Be aware of and comply with legislation and organisational requirements relating to Workplace Health and Safety
2. Client and stakeholder engagement
• Make relevant referrals, broker services and deal with suppliers as per care plan
• Develop and maintain relationships with community organisations to ensure a holistic case management approach
• Develop and maintain relationships with brokerage agencies to ensure quality services are delivered by external providers.
• Ongoing communication with clients, carers/families to ensure they are aware of rights and responsibilities to make informed choices.
Clinical Component of the Role
We are looking for an RN that has sound clinical and problem solving skills and has the ability to undertake health assessments (RN reviews, wound, diabetes, continence) . In the position you will review and sign off on all care plans
You will be involved in the care of our complex clients, providing clinical oversight. You will provide education and support for our Support Workers and Case Managers so that we are providing the best client centered care that we can. .
Some travel will be involved in the position as we service clients from the Gold Coast to Lismore
Competencies
1)Demonstrated ability to work with individuals, families and carers from a strength based approach, using high level interpersonal, verbal and written communications skills, together with negotiation liaison and conflict resolution skills
2) Demonstrated understanding and experience of current case management practice and principles such as assessment, developing and reviewing care plans, making referrals.
3) Demonstrated ability to develop quality individual plans that are consistant with principles of person centered planning within CDC frameworks.
4) Computer literacy skills and experience working with client management software systems.
5) Knowledge of local and regional networks and service providers

Requirements:
- Qualified Registered Nurse with preferably 2 – 5 years experience
- Ability to work collaboratively in our multi disciplinary team
- Relevant case management experience supporting clients with high complex needs
- Confidence in managing your time effectively, including an ability to work autonomously and part of a team
- Well developed interpersonal skills that you use to establish excellent working relationships with both internal and external stakeholders
- Blue card and NDIS Workers Screening Clearance or willingness to obtain
- Full, unrestricted AHPRA registration and legal working rights in Australia
- First Aid and CPR
- Current Australian Driver’s Licence and, as you will use your own vehicle, proof of your comprehensive car insurance.
