Employing over 300 dedicated staff, they provide high quality residential aged care for more than 200 men and women in 5 homes across Northern Sydney – Gordon, Mosman, Dural, Hunters Hill and Gladesville.
The Role
A Clinical Manager is responsible for the provision of quality care to residents. Your duties include:
- Clinical assessment and Care planning, organise/schedule care conferences, manage care conference, manage specialist referrals
- Liaise with allied health professionals
- Following up on residents clinical investigations and reports, including RMMR, Allied Health, Radiology and Pathology results
- AN-ACC reviews
- PACOC profile and outcomes
- NQIP data collection and clinical indicators monitoring and analysis
- Identifying high risk residents through FMOC and care circles
- Supporting resident choice and decision making
- Education and mentoring staff
- Following AHPRA Registered Nurse Standards for Practice
- Following legislation requirements for incident management and restrictive practices
- Participating and evaluating moving on audits
- Collate the NQI data to ensure it is submitted within the legislated time frame
- Drive Continuous Improvement and quality activities based on contemporary evidence
- Updating the PCI and Self Assessment as required
- Ensuring that education programs and quality improvement initiatives are implemented and comply with Legislation and the Aged Care Quality Standards and NDIS Practice standards and Quality indicators
- Communicating with relevant parties
- Lead the clinical huddles and actively participate in RN meetings
- Daily review and follow-up of the progress notes and handover.
Details
- Working at our home in Mosman
- Working hours will generally be 9-5, Monday to Friday, with potential flexibility at the convenience of both employee and the home
- Handover with current Clinical Manager
About you
Demonstrating experience in a similar role, you will treat residents in a respectful and courteous manner. Further, you will have:
- Current Registration with AHPRA
- An up to date Police Check
- NDIS Clearance
- Previous experience working in community or aged care would be highly regarded
- Well-developed written and verbal communication skills, and interpersonal skills
- Work at any Twilight home as per operational requirements of the organisation
- Engaging in formal and/or informal discussions and catch ups with staff
- Leading and supporting the clinical team on a day to day basis
- Participating in staff performance management process and/or performance improvement plans
- Maintain up to date knowledge in areas of practice and responsibility and participate in own professional development as per APHRA and organizational requirements
- Assisting compliance with legislated 24/7 RN care requirements, including performing RN shifts if necessary
