About the business
Fitzgerald Aged Care has been especially designed for the enjoyment of older people in the Hawkesbury who can access help with day-to-day living while retaining their independence. A total of 48 residents can be accommodated at this beautiful venue which, features spectacular views, fully equipped kitchen, dining rooms, large common areas, balconies and gardens. The facility maintains 46 permanent residents and 2 respite residents. This project would not have been possible without the support of the Hawkesbury community.
This role is expected to ensure our residents receives both personal care and clinical care, that is safe and in accordance with the resident's needs, goals and preferences to optimise health and wellbeing.
About the role
Key responsibilities:
- Lead and guide registered staff and care employees to ensure that the culture reflects an inclusive environment which puts the residents and families/representatives' needs first
- Review of care incidents and care change patterns for residents and determine if further assessment and care planning modifications are needed, initiate care plan reviews and re-assessments
- maintain accurate client records, case notes and other data as directed and required.
- Monitor the delivery of care is consistent with the care plan and goals including dignity of risk choices and take action as required if gaps or inconsistencies are identified
- Liaise and negotiate with regulatory organisations including: Aged Care Quality and Safety Commission, Government Departments, Trustee and Guardianship Boards;
- Ensuring "best practice" medication management guidelines are observed and regularly monitored
- Responsible for the completion of regular analysis of the use of psychotropic, antipsychotic, polypharmacy and antimicrobial stewardship and reporting and consent obligations.
- Actively identify opportunities for ongoing continuous improvements to policies, procedures, work instructions, and the workplace environment;

Skills and experience
The Skills, Knowledge and Experience required:
- Current nursing registration with the National Nurse's Registration Board (AHPRA)
- A minimum of two years' clinical management experience within aged care with demonstrated clinical leadership skills
- Demonstrated knowledge and experience applying the Aged Care Funding Instrument (AN-ACC) and knowledge of the Aged Care Act and Accreditation Standards;
- A high level of knowledge and understanding of the Care Planning, Assessment and Documentation in Residential Aged Care
- Excellent and demonstrated verbal, written and interpersonal communication skills.
- Knowledge and ability to use computers and technology efficiently.
- Current Police Check and appropriate Working Rights
- Driver’s License with a car
- Up to date vaccinations (Influenza, Covid-19)
- Experience in a similar position in Aged Care - desirable
- Management qualifications - highly regarded
