Job description

We have an exciting opportunity for a Commissioning Coordinator to join our future focused and rewarding organisation.  This role reports to the Director – Mental Health and is a key member of the Commissioning Team. 

Darling Downs and West Moreton PHN is a not for profit organisation that works at a regional level to achieve improved whole of system health care. 

Our key objectives are to:

  • increase the efficiency and effectiveness of health services for patients—particularly those at risk of poor health outcomes, and
  • improve coordination of care to ensure patients receive the right care, in the right place, at the right time.

Our PHN achieves this by working with local health care stakeholders and community to:

  • fund programs and services based on local health needs;
  • support primary health care professionals, like GPs, general practice staff and allied health professionals, to improve the care they provide to their patients; and
  • support the development of a local sustainable health care workforce.

About the role:

This role will contribute to the design and delivery of the Darling Downs and West Moreton PHN commissioning responses within the Mental Health, Alcohol and Other Drugs and Suicide Prevention Program areas; ensuring that responses are aligned to the strategic plan, are evidence based, meet population health needs, and address service gaps, equity, and access issues, to improve health outcomes.    

Key purpose of the role:

  • Contribute to the development, implementation, monitoring, and evaluation of regional and place-based strategies to improve health outcomes in response to regional priorities.  
  • Develop and deliver projects and programs in collaboration with internal and external stakeholders, underpinned by evidence-based project management methodologies and an agile response.   
  • Build and maintain collaborative relationships with key stakeholders internally and externally, including other PHNs, Hospital and Health Services and local service providers, to ensure commissioned services and initiatives are integrated, meet high priority population health needs and result in better outcomes for the community.  
  • Work collaboratively with a wide range of stakeholders to identify service gaps and access issues and develop evidence-based solutions to address these in collaboration with health system partners, fostering integration at the system level.  
  • Lead the development and implementation of innovative evidence-based service delivery models and system responses in collaboration with key stakeholders.  
  • Utilise appropriate mechanisms to ensure strong governance throughout the commissioning cycle, ensuring evidence informed program and procurement planning, contracting, service implementation, monitoring, learning and evaluation.  
  • Actively participate in and inform monitoring and evaluation of commissioned programs and activities.  

Selection Criteria:

  • Relevant tertiary qualifications and/or experience in health, human services, or relevant related field.  
  • High level knowledge and understanding of contemporary evidence-based service delivery models, planning, strategies, and guiding policy documents across the Mental Health priority.
  • Demonstrated knowledge of the primary health care and community services sectors. 
  • Demonstrated ability to partner and collaborate with a wide network of individuals and organisations to achieve meaningful outcomes.  
  • Excellent written and verbal communication, interpersonal skills, time management skills and project management skills with demonstrated experience managing and evaluating complex projects with a diverse range of stakeholders.  
  • Ability to build successful relationships and partnerships with key stakeholders both internally and externally. 
  • Strong analytical skills, with the ability to synthesize, interpret and translate complex qualitative and quantitative data into accessible briefs, concepts, plans and reports.
