The Eyre & Far North Local Health Network (EFNLHN) is seeking a dedicated and skilled Community Care Coordinator to lead our comprehensive Community Care Program. This role is crucial in identifying needs, assessing, implementing, coordinating, maintaining, and evaluating home and community-based programs, ensuring they meet the highest standards of care and support.
As a Community Care Coordinator, you will collaborate closely with a multidisciplinary team to develop, implement, coordinate, and evaluate community-based ageing and disability programs. Your role will focus on empowering individuals who wish to continue living independently in their own homes by providing essential assistance and support services.
Country Health Connect focuses on the delivery of exceptional care to those living within our communities who require some support to remain living independently or reside in one of our residential aged care facilities.
Our professional and caring staff deliver health care and wellbeing support across regional South Australia in hospitals, local health services, community centres, residential aged care facilities and clients’ own homes.

Applicant Information
This is a part time vacancy, working 52.50 hours per fortnight and salary will be adjusted according to actual hours worked.
