About the organisation:
Aboriginal Community Services (ACS) are a not-for-profit provider of residential aged care and home and community services to Elders / Clients across South Australia.
About the department:
The major objective of the Client Services Team is to assess, link, coordinate and deliver a range of services that includes, but not limited to, personal care and nursing, domestic assistance, meals, social support, transport, allied health options. The team works with clients who receive services under the Commonwealth Home Support Program (CHSP).
About the position responsibilities:
- Manage client services in accordance with the CHSP guidelines and legislation to promote and achieve positive client outcomes.
- Develop, implement, and review client service/care plans within timeframes as per legislation, ACS policies and procedures.
- Internally and externally referring to and liaising with medical and allied health professionals.
- Intake and assessment of new clients.
- Undertake annual review at minimum of client care needs, complete required documentation and processes within timeframes as per legislation, ACS policies and procedures.
- Regularly evaluate client care and supports and monitor changing or emerging needs.
- Connecting clients with external agencies/stakeholders to provide additional supports that are outside of the funding guidelines
- Awareness of risk in a community setting, develop strategies and intervention to safeguard client and workers.
- Assist with referrals from Regional Assessment Services and ACAT in a timely and coordinated manner.
- Actively participate in a Duty Roster / On Call Roster
- Direct support to Community Support Workers in their day to day operations
- Provide training and support to new staff
- Self auditing documentation compliance of documentation, processes and reviews
- Incidents (SIRS Reporting), feedback, complaints and improvements reporting to be created in an accurate and timely manner and in accordance with legislation, ACS policies and procedures.
About you:
To be successful you will possess excellent communication and organisational skills. Your strong multitasking skillset, attention to detail and accuracy and ability to work in a fast-paced environment will see you succeed in this role. You must possess unrestricted work rights in Australia and a full Driver's License. Previous experience working with Aboriginal People will be highly regarded but is not essential.
