Experienced Home Care Services Coordinators (38 hours per week) based in the following APY Lands' communities:

• Amata SA 0872 (1 position)
• Fregon SA 0872  (1 position)
• Mimili SA 0872  (1 position)
• Indulkana SA 0872  (1 position)

Free shared housing accommodation comes with the job.

Generous salary packaging is available of $15,899 and an additional $2,650 on meal entertainment and vacation accommodation potentially increasing your take home pay by $6,399 per annum.

Aboriginal Community Services (ACS) is an ACCO non-profit provider of residential aged care and home and community services. We provide Aged Care services to our Elders / Clients across South Australia in metro, rural and remote locations. For more info about ACS visit https://aboriginalcs.org.au/

Position responsibilities:

• Intake of new client (coordination).
• Develop, implement, and review client service plans.
• Manage client services in accordance with the NATSICFAC, CHSP and HCP guidelines and legislation to promote and achieve positive client outcomes.
• Develop and maintain regional service resources to maximise service provision and support clients.
• Assist with referrals from Regional Assessment Services and ACAT in a timely and coordinated manner.
• Develop strong collaborative relationship with other ACS staff / programs and external agencies to deliver seamless and integrated client services.
• Actively participate in a Duty Roster.
• Advocate for increased services in accordance with identified unmet needs or health deterioration.
• Provide incident / injury management support to Community Service Workers and contractors.
