About the role:
We have an exciting opportunity to join Colbrow Care’s case management team as a Case Manager servicing the Greater Melbourne Region. The office is situated our brand new purpose built facility in Hawthorn.
The Community Care Case Manager is responsible for the performance of consumer assessments, case management, clinical care and services for consumers receiving in-home nursing and care, whether packaged or private. The Community Care Case Manager RN will report to and be accountable to the General Manager and the Clinical Educator and provide supervision and training to care staff, where required. The Community Care Case Manager RN will liaise with the Care Coordinators and Clinical Educator on a regular basis so that he/she is appraised with any issues and developments arising within the consumer base.

Candidates will need to be a Registered Nurse.
Responsibilities includes:
- Co-ordinate clinical and other activities to deliver service efficiency and effectiveness as well as regular, service reporting and continual enhancement of services in line with continuous improvements principles.
- Adhere to all requirements and obligations of the Aged Care Act plus NDIS and associated Regulations and Standards in the delivery of care.
- Deliver clinical and other care and services to clients using a Consumer Directed Care approach.
- Participate in Clinical Governance matters arising and committee
Essential Criteria:
To be successful in this role, you will need to demonstrate the following:
- Registered Nurse with AHPRA registration.
- Experience in Community Nursing and Case Management
- Experience with Acute care
- Proficient with internal databases, Microsoft office and government report funding portals
- Current Victorian Driver’s Licence (which must be maintained throughout the course of employment)
- Current NDIS Worker Screening Check
- Current Working with Children's Check
- Current First Aid Certificate (HLTAID011) and CPR (HLTAID009)
- Valid working rights in Australia
- Mandatory competencies (as required by Colbrow Care)
