Based in Hervey Bay, Maryborough and/or Outreach Clinic {Fraser Coast, Qld} - Working together to help “Close The Gap”.

We are a Community Controlled Health Organisation (ACCHO) established in 2007. With multiple clinics including an outreach service and growing across the Wide Bay region, we currently provide health care services for more than 3,000 Indigenous residents. 

Our Board of Directors represents the communities we serve, ensuring every decision we make helps equip local people to take control of their lives and pro-actively manage their health.

We have a position available for a Community Connector. This is an exciting opportunity to join a team committed to make a direct impact on improved health outcomes in the Aboriginal and Torres Strait Islander community.

Our purpose-built clinics in Hervey Bay and Maryborough offer exceptional facilities. We have K’Gari (Fraser Island) our natural wonderland and biggest sand island in the world in our Region for day or weekend visits. The Fraser Coast Region is located 2 hours to the Sunshine Coast and 4 hours to Brisbane both reasonable driving distances.

As a member of a multi-disciplinary team, you will ensure high standard of customer service, effective and efficient administration of client appointments, client data and client claim processing are delivered in accordance with our Model of Care. Plus, the provision of general administrative support to the Clinic and Program Teams.

Key Duties include:

  • Using the electronic patient data base systems including but not limited to, Best Practice, Proda, CTG/PIP, eHealth and Medicare systems:
    • coordinate patient registration, appointment and contact details
  • Support implementation of Connected Beginnings strategic plan and actions, monitor outcomes and record results.
  • Participation in line management meetings with the aim of monitoring performance, providing support, and fostering continuous improvement.
  • Reporting to Managers on actions and outcomes
  • Understanding of children’s services, funding, and gaps in current service delivery environment.
  • Identify continual improvements for the programme and provide solutions informing improvements to the action plan.
  • Attend networking and local level meetings representing the programme’s and Galangoor’s interests.
  • Identify and communicate gaps, needs and solutions for families to engage with health, education, and family services.
  • Culturally sensitive event planning and management
  • Demonstrated links and experiences working in or with local services and support the development of partnerships with health, family and childcare services throughout the Fraser Coast.
  • Ensure Galangoor has a positive reputation in the region.
  • Support Galangoor’s appropriate participation in community events and participation of community in Galangoor events and initiatives
  • Represent Galangoor at appropriate forums and networks.
  • Building partnerships with local Aboriginal organisations and community groups that hold cultural authority, to support and elevate community leadership ensuring Aboriginal people lead the decision making.

How do you know if you’re the right fit? 

We are looking for people who are adaptable and understand that empathy is needed to build strong, mutually satisfying relationships – whether it’s with patients, colleagues, community partners or our community elders, aunties and uncles.

Before you reach out and apply, we need you to have:

  • Certificate 111 in Aboriginal and/or Torres Strait Islander primary health care or equivalent (not essential but preferred; or willing to work towards)
  • Ability to effectively work within a diverse interdisciplinary team which includes a range of clinical team, non-clinical team and community members 
  • Exemplary interpersonal communication skills – written, verbal and interpersonal
  • Commitment to patient centric care
  • Excellent data entry skills and meticulous attention to detail
  • Recommended Immunisations as per Galangoor's Practice Team Immunisation Policy 

It would also be beneficial if you have:

  • Experience in Human Services, Community development/engagement, Early Childhood Education or relevant experience desirable, but not essential.
  • Demonstrated knowledge and understanding of the social determinants of health issues affecting Indigenous people and how Primary Health Care principles apply in an Aboriginal Community Controlled Health setting.
  • Commitment to and understanding of the characteristics of Aboriginal Community Controlled Health Organisations.

While knowledge of Aboriginal and Torres Strait Islander culture is helpful, what will really see you thrive in our community is an open mind, willingness to be exposed to culture, and curiosity.
